Description Of Incident: Fire on 6th floor of a 13-story high rise project type 140X100 building "The Beach 41st Street Houses."

1353hrs- 10-75 for fire on 6th floor of a 13-story OMD.

Original Box E-265, 264, 328; TL-121, L-134, B-47.

On the 10-75: L-137 FAST, E-266, R-4, Sq-270, B-54, Div. 13

1355hrs- 10-77 Transmitted. L-173, TL- 107, E-268 (CFRD)

B-50, B-39 (Safety), Rescue B/C, Safety B/C, E-3/High Rise 1; Field Comm 1; TAC 2, B/C 43 for Air Recon.

1358hrs- B-47 rpts Using All-Hands for heavy fire in a 6th floor Apt. (1) L/S going into operation, 2nd underway. Trucks opening up. D/W/H.

1403hrs- B-47 Transmitting Urgent 2nd Alarm with May-Day on fire floor.

2nd Alarm: E-329, 331, 311, 285; TL-142, TL-155; E-291/S-4; B-51 (Com Cord)

1410hrs- All firefighters accounted for. (1) injured member from L-121 with burns.

1415hrs- Co's will be deploying fire blankets over windows of fire apartment. Ladder co's cutting down fences to access yard to position rigs below fire floor.

1417hrs- Div. 13 rpts heavy fire blowing out the Apt. into the hallway. Co's are moving in at this time with window blanket protection deployed.

1427hrs- Div. 13 req. 3rd Alarm with S/C (3) additional Trucks above the 3rd.

3rd Alarm: E-314, 209, 323, 275; TL- 120, TL-170; B-58, B-33 (Staging)

S/C Trucks: L-158, L-133, L-126

1429hrs- S/C (2) additional Squads (288/252) for window blankets and (2) additional Truck Co's above 3rd Alarm.

1432hrs- Div. 13 rpts We have members trapped in Apt's on the fire floor.

The hallway is untenable at this time due to fire blowing out of the original apartment.

1445hrs- (1) ladder pipe set up and in operation. Fire blankets have been deployed.

1506hrs- Car 9 rpts all visible fire on 6th floor is K/D. Still have smoke condition on upper floors. P/W/H.

1510hrs- EMS transported (2) members to Nassau Medical Center Burn Unit. (3) additonal firefighters being treated at scene ATT.

1524hrs- Div. places the fire Under Control.
