1014hrs- Original call for alarms ringing with odor of smoke. Additional calls for smoke in building.

--1st Alarm: E-313, 311, 307, 314 L-73, 75, B-2, R-1

1017hrs- B-2 reports 10-29 fire on 2nd and 3rd floors of 4 story OMD. Req. 4th due Engine (314) to stage on Midland Ave and report to front of building as FAST.

1019hrs- B-2 req. 2nd alarm with an additional truck above.

--2nd Alarm: E-303, 306, L-70 additional truck co. L-74

--Size up: 4 story brick "U" shaped OMD, fire in 100X400 west wing. Complex is a gated community with narrow access roads.

--Exposure 1: street, Exposure 2: yard, Exposure 3: parking lot, Exposure 4:courtyard

1021hrs- L-70 to the roof via TL-75. B-2 reports fire on 2-3-4th floors, 4 L/S/O.

1031hrs- B-2 req. 3rd alarm for fire into cockloft.

--3rd Alarm: E-309, 312, L-72

1035hrs- L-74 to enter via gate on Lincoln Ave and use access road to 1/4 side. L-70, 74, R-1 working on roof. All co's working.

1040hrs- E-312 to enter from Midland Ave on to service road I/R/O and feed E-313 in rear. Heavy smoke from 3rd and 4th floors and from eves. Additional lines to 4th floor. Heavy fire in cockloft, co's continue to open up.

1045hrs- B-2 req. 4th alarm. Fire thru roof. co's with num. lines in op. in building.

--4th Alarm: E-304, 310, TL-71

--Mutual Aid: Mt. Vernon E-2 (Sta. 13), L-2 (Sta. 1), New Rochelle E-21, L-12 to Sta. 12

1055hrs- B-2 req. 5th Alarm filled out (E-308 only). Co's making progress. No further extension into west wing other than fire apartments and cockloft.

1120hrs- Main body of fire K/D. Spare apparatus in service with recall manpower. (E-302, 305, L-77)

1200hrs- Req. E-311 to quarters to respond to scene with Collapse Unit.
