2141hrs- 4 engines, 2 trucks, Rescue assigned due to calls. Squad 41 rpts 10-75, heavy fire conditions. Normally assigned Squad is 1st due engine co.

2143hrs- 2nd Alarm on arrival of B/C.

2153hrs- Staging Area- River Ave and 149 Street

2156hrs- Fire is through the roof. TL-17 setting up in front. Div. 6 reports all companies going to work including all 2nd Alarm co's.

2201hrs- Progress Report- 4 (2 1/2) lines in operation, co's opening up, using all hands.

2209hrs- 3rd alarm transmitted

2210hrs- Asking for additional tower ladder

2215hrs- All co's pulled out of building and going to exterior op's. (2) TL's set up in front, (2) TL's setting up in rear.

2230hrs- All co's moved away from front of fire building due to risk of collapse. Members removed from tower ladders. (1) multi-versal only in op in front A/T/T.

2256hrs- (4) TL's operating at this time.

2310hrs- Still heavy fire through the roof.

2340hrs- 4th Alarm transmitted by orders of Car 13B (Deputy A/C of Dept.), 4th alarm co's to protect exposures.

0033hrs- Car 13A reports still heavy fire throughout with no ext. to exposures. (4) TL's in operation. Co's have lines in operation on roof of Exp. 2 and Exp. 4 with partial collapse of front of building.
