Why is Getting and Maintaing NYC REMAC so Difficult?

Just venting here a bit...

Seriously, is there any reasonable explanation as to why it needs to be such a drawn out, painful, difficult, and EXPENSIVE process to get and maintain a city MAC?

And why such obscure protocols? Do people's physiology change when they enter one of the 5 boroughs or something? I just don't see what's wrong with using the same, industry wide accepted, treatment modalities as the rest of the state.
