0807hrs- 10-75 for fire 2nd floor of 4-story brick OMD.

0830hrs- 2nd Alarm transmitted by Div. 7, reports fire on 3rd and 4th floors and in the cockloft of a 4-story 40 X 100 brick. 3 lines in operation and reports of a Mayday.

0834hrs- 3rd Alarm by orders of Div. 7. Fire out windows on the 4th floor. EMS w/rush for several firefighters down, with numerous Maydays.

0843hrs- EMS reporting (5) critical, including (1) in traumatic arrest, MCI declared. (6) members injured from jumping out 4th floor window after possible flashover.

0910hrs- Div. 7 reports 3 lines in operation, main body of fire is K/D still D/W/H. (6) firefighters injured with (1) likely to die. Nimerous EMS units and NYPD ESU treating injured. NYPD ESU and Wighway setting up escorts to Jacobi and Barnabas Hospitals.

1150hrs- EMS responding to Quarters of E-94 for a firefighter injured at 3rd Alarm.
