Description Of Incident: Fire in a 6-brick OMD H-type 60X60 with extension to a P/D.

2350hrs- 10-75 for fire on the 5th floor.

1st Alarm: E-328, 264, 266, 265; L-134, L-121, 137 FAST; B-47, B-54 (10-75); R-4, Sq-270.

2357hrs- B-47 rpts Using All-Hands for fire on the 5th floor of a 6-story brick OMD H-type. (2) L/S, (1) in operation. S/C extra Engine and Truck.

S/C: E-314/158

0001hrs- B-47 rpts (1) 10-45. L-137 going to work, req. L-158 to come in as FAST.

0003hrs- B-47 using E-314 as FAST until L-158 gets in.

0011hrs- B-47 rpts fire is in Apt. 5H.

0013hrs- Div. 13 transmitting 2nd Alarm for fire into the floor above.

2nd Alarm: E-268, 329, 311; L-173; E-324/S-4; B-51 (Safety), B-50 (RUL); Field Com., TAC #2, Rescue B/C, Safety B/C.

0020hrs- Div. 13 transmitting 3rd Alarm for fire now into the cockloft with heavy fire conditions on 5th and 6th floors. Staging area will be Mott St. X Cornaga Ave

3rd Alarm: E-331, 302, 225, 293; L-155, L-107; B-53, B-43 (Air Recon), B-39 (Staging), MSU.

0022hrs- Div. 13 rpts members trapped on top floor. Units in process of taking roll calls.

0027hrs- Div. 13 S/C (2) extra Trucks. L-133 will be new FAST. S/C additonal Rescue (2).

0032hrs- Div. 13 rpts heavy fire throughout 5th and 6th floors of the "B" wing and into the cockloft.

0038hrs- Div. 13 rpts co's going to exterior operation with a Tower Ladder on the 6th floor. Co's being removed to floors below. Roll calls still being conducted. All 1st Alarm co's accounted for.

0048hrs- Div. 13 transmitting 4th Alarm for fire on the 2nd through 6th floors and through the roof. All members accounted for. Setting up exterior op's. S/C Squad 288.

4th Alarm: E-236, 315, 308, 294; L-111, L-124

0054hrs- Car 15 trasmitting 5th Alarm.

5th Alarm: E-309, 275, 317, 301; L-165, L-150

0104hrs- Car 15 rpts heavy fire conditions 2nd through 6th floors and cockloft. All members withdrawn to the 2nd floor. (1) Tower Ladder and (2) large streams in operation. R-2 performing searches in the "A"-wing.

0122hrs- Queens going to Fall Back Step #3. S/C (2) B/C's.

0129hrs- Fire has extended into the "A"-wing top floor and cockloft. Co's making progress on floors 2-5. Still heavy fire top floor and through the roof.

0130hrs- Car 15 transmitting 6th Alarm. S/C (1) B/C.

6th Alarm: E-326, 298, 305, 299

S/C: B-45.

0131hrs- Re-directing B-46 to 931 Dinsmore St for P/D fire from embers. Units at scene of 6th Alarm being assigned. Address is (1) block north of the fire.

0134hrs- Car 4 S/C Truck (143) for FAST at 931 Dinsmore St. Box is (3) Trucks above 6th Alarm.

0141hrs- Heavy fire 6th floor "A"-wing.

0148hrs- L-143 re-directed to the staging area.

0158hrs- Car 4 rpts (3) Tower Ladders in operation, (2) on Exposure #1 side, (1) on Exposure #2 side. Setting up TL on Expsoure #3 side. Collapse zone established and all members out of the building and standing fast on Exposure #3 side.

0230hrs- (4) Tower Ladders and multiple lines in operation from exterior.

0342hrs- P/W/H. S/C E-330 with Thawing Apparatus.
