Carmel FD mutual aid with one Tanker ( 12-4-1 ) to the scene. 0806hrs- M/A Westchester for Engine and Tanker. Croton Falls resp. with E-148. 0811hrs- Goldens Bridge resp. w/Tanker 1. Croton Falls resp. with R-28 and additional manpower. 0820hrs- Putnam County Fire Investigation Team requested to the scene. 0825hrs- Additional Manpower from Brewster requested, along with BFD EMS for standby, and BFD Heavy Rescue to the scene. Structure is a private dwelling, unknow if occupied or vacant. 0911- All Mutual Aid companies are returning to there respective quarters. I havent heard an update on the actual fire would have to guess it is knocked down.

Using both MFD ambulances and 1 from Mahopac Falls FD. MVFD also on stand by with second ambulance. IC- 18 requested second Medic to scene. 1845hrs: Medic 3 and 18-7-1 enroute to Putnam Hospital 1847hrs: MVFD 19-7-1 Enroute to Putnam Hospital and requesting a intercept with Putnam Medic 4

No extrication was required, 2 patients transported by 17-7-1 to Putnam Hospital (BLS ).

Address is a group home for troubled teens. Havent heard a lot of talk on the radio except that they have paged several times for additional manpower. No mutual aid has been dispatched as of yet.

Date: 11/17/04 Time: 2130hrs Location:Routes 311 & 52 City/County: Lake Carmel, NY ( Putnam County ) Agencies: Listed Below Frequency: Writer: Nicholas994 Car into the water Units Operating: 17-1-2 Incident Command 17-1-3 Operations LCFD 17-2-1, 17-4-4, 17-3-3 (and boat ), 17-8-1, 17-7-1, Mahopac Falls 19-6-1, 19-1-1, 19-7-2, and Dive Team Carmel 31-7-1, Patterson 22-7-1 Putnam Medics 3 and 4 Putnam County Car 5 NYC- DEP Hazmat Aparantly driver drove off the road for unknown reason and rolled vehicle into about 10 feet of water. Victim was rescued by LCFD members, and transported to Putnam Hospital with 17-71 and P.C. Medic 4.. Mahopac Falls Dive Team conducted a search, no other victims where found. At 22:30 hours vehicle is being removed from water, Mahopac Falls is being released
