Lately it seem to me that good BLS has fallen by the way side and the lack of skills on the EMT part and some ALS providers have been lacking in BLS skills, I have been on calls where the BLS crew waited 10 plus minutes for the medic to get there before they take a blood pressure or start taking a history on the patient???? the EMT just walking in with the clipboard on someone with difficult breathing still doesn't help the patient. I have seen other bring every single bag out of the ambulance inside when the medic is already there and treating the patient and the bags just get in the way because they drop them and go get the stretcher... does anyone have an idea on how to change this?

narcan sure isn't going to fix it. that is like a finger holding back a dam when hurricane is coming something should be done
-
how about instead of giving out Narcan they cops start cracking down on this s*** and arresting the hell out of people for it. its been going on too long up here in northern westchester for way too long. i remember back in my high school days when it was getting bad and kids were doing it in schools in between classes at a VERY well to do high school too. it needs to be put under control before it gets worse then it is... and the rate its been going its going to be bad.

well i don't blame the hospital for doing this.. hell i would. but why didn't the insurance company of the driver of the car that hit hear pay her medical bills.. thats why we have car insurance. and to cover pain suffering, and medical bills you can also go after the driver. only makes sense that the hospital get paid if your going to sue someone on the grounds of medical bill payments... ?

I have personally been caught in a couple of these scenarios and some one ones too, to cya i stopped, checked things out and called for another unit if i had a patient and my patient was stable. when my patient wasn't stable... make sure there is nothing life threatening and at worst leave my partner and ask a cop or something to drive the bus to the hosp. The new situations i have been getting into is where there are no medics available in a fly car system all on minor BLS calls and a true ALS call comes in. You can't leave the scene because you and the cops are the only ones there... they need to peel off and go to the serious call now and you are left there waiting? what do you do? and the ambulance goes to the higher call and don't take the 1st call which is a minor BLS call? while they scream for a medic at the other job.

yeah but that won't save too much money. you just need some place that patrols can turn out of, you don't need holding cells dispatchers, maybe some computers desks locker rooms and what not but not a whole station. Do it like the state troopers do, a couple of the barracks have holding cells i am sure there is room at the county jail for them too lol. I am sure a couple of the station or what not can be combined to also cut some costs but that will take years and i should be denied my pension by the time that starts to come to paper and pen But this is a good start to cutting some of the LARGE amount of fat that is in Westchester county. hope it all works out and the community doesn't notice a change in services

the water to the pumps are supplied by a "sea chest" a large opening that is capped off under the water line usually going through the hull in an engineering space. typically there are several pumps and the bilge systems and also be crossed linked with the fire pumps. this is to make stabilizing the ships easier and a faster evolution. but flowing water on deck is alot different then flowing water below deck stability is key and necessary to maintain, so there are alot of fire extinguishers.

i am not a tactical medic but i do work with the navy on my A job and have been trained on a couple of weapon platforms some very large. and its seems to be easier to learn then being a medic. Yes we can't enforce laws, but why can't they be a "swarn" person or consider a shooter in and only when the full team is formed and when needed based on the situation also. I am sure this is breaking 150 years of tradition or something, but i am not too sure how many medic would go in to a situation without at least a 9mm or something.

well workers that go into sewers and other small places underground that are very poorly ventilated actually have or suppose to have SCBA and gas detectors and other proper equipment for entry into the space. if you read up on what happened in tarrytown alot of towns are checking to see if they meet standards now. Its a shame it took two lives to be lost to make this happen

AMEN

good idea but it will never happen because of too many politics and too many 911 answering/dispatching sites in westchester at last count i beleave its somewhere around 58... and nyc is 1?

I have been down there alot lately and have seen trucks and stuff that looks brand new too... there is a scene support truck too? looks like a 10 wheeled modified box truck with a 4 person cab... i can't say for sure i didn't take a really good luck at it... but it would be nice to know what is there if we need it... why can't the tech rescue guys or hazmat guys staff it as a volunteer thing? just get it there type of thing?
-
with system management... aka post on the street corner keep the truck on and keep the tank full.. you don't want water in there

You actually got me thinking when you mentioned cyanide treatment/antidote.... why can't we be more proactive and get the Taylor kit for ALS providers. One way we can offset the cost of the kit can be applying for a grant as a drug against WMD. But that would have to be done by someone a little more tactful and good with grant writing. But this might be an interesting read though. CO Treamtment in the Field

Its nice to know that Phelps does have a chamber.. but from my understanding the main reason they purchase the chamber and put it into service was for wound treatment.Even if this happened while the chamber was open and staffed... do they have the trained personnel to handle an unstable patient in the chamber? I am not too sure whether or not the medical center's chamber is for wound treatment and or emergency CO or dive injuries?? The only true chamber I know that is for Emergency use is Jacobi's, which is a trip from the Northern Westchester Area. In my experience high concentration of O2 have done a lot over hours for a patient.. aka NRB then step down to a nasal canual.

paper work vs the amount of pain someone is in. ice pack work wonders

The Engineer is a licensed officer usually a limited horse power license by the USCG. A wiper is an unlicensed working in the engine room. Wiper is a promotion from FF. Engineer is a promotion from Wiper.

ok does any of the marine units take basic and advance marine fire fighting at the US Navy training center in NJ?
-
sounds great but i thought the FD guys were EMT-Is did they upgrade? are they available for mutal-aid?
-
The patient needed RSI. and only 2 agencies in the county have it.
-
I don't know if it me or I am hearing things.. but the patient seems to be complaining about something when the medic trys to get into the back of the amb and the cop keeps pushing his finger into the medic's chest.. but it did look like the medic was going to get into the back for some reason.. and then someone else does go in.. well were all weren't there but I have seen some weird things lately and it seems to be increasing and spreading eastward too. I wonder how this have would have played out if it was something a little more serious like a cardiac arrest.. god forbid a pediatric cardiac arrest i don't think the cop would have been able to get me to stop unless they pitted the ambulance or cut me off

There and in it self is the problem.. you would have to find doctors that understand what a paramedic does... and then want to actually promote what they do and help them do it. I have only found a few that are like this, and greatly enjoy it when they are working.
-
i wish westchester would get CPAP or RSI.. or something anything would help..
-
i was there last wed.. they lost it was a close game but puts needs to learn how to throw a strike.. yea people were drinking just before the entrance actually.. as long as you don't go in with it they don't seem to mind.. pd was kinda lax too i did see a couple of tailgaters though.. wish i was one of them

Go for it.. more help the better isn't that what EMD was made for?
