Date: 12-6-05 Time: 0640hrs Location: 753 Revere Ave X Phillip/Randall Frequency: Units Operating: NYSP, Yonkers PD Special Investigations Unit, NYPD ESU Truck 4, Truck 3, NYPD Aviation Description Of Incident: (2) members of NYSP Tactical team (SWAT) shot while conducting a warrant service (raid) on a private house. Writer: Truck4 0640hrs- NYSP Tactical Team executing search/arrest warrant as part of simultaneous raids of suspected gang members' residences which included locations in Yonkers and Hastings On Hudson. As the team began to force entry, shots were fired through the door by the suspect, striking (2) tactical team members, possibly striking a ballistic shield first. Once door was opened, suspect continued firing and was put down by tactical officers. 0700hrs- (2) NYS Troopers transported to Jacobi Hospital- (1) GSW to the leg with entrance and exit, (1) with GSW to the temple and chest- with the rounds striking ballistic helmet and vest. Both officers have non-life threatening injuries. Numerous NYPD ESU responding. 0720hrs- Perp is confirmed DOA. NYPD ESU req. Bomb Squad for possible devices in residence, including a possible live grenade. Numerous NYSP units also responding. 0800hrs- NYPD Avaition req. to scene to remove numerous news helicopters. Additional locations in Yonkers and Hastings secured without incident and multiple arrests made.

Witness states a passer-by had jumped in after the car and cannot be located. 1815hrs- M/A Mount Sinai and Rocky Point FD Dive Teams to scene. 1816hrs- M/A Setauket FD for boat. 1820hrs- M/A Terryville for Rescue to scene. 1826hrs- PD reporting (2) victims out of the water. Units trying to determine actual number of occupants in car. Vehicle is submerged. 1828hrs- M/A Stonybrook FD boat to scene. PD reporting car occupied by (2) and (1) bystander who jumped in the water.

Description Of Incident: MVA with pin and multiple injuries. Writer: Truck4 1500hrs- Roadway remains closed for accident investigation. MVA involved a flatbed work truck that lost it's brakes, striking (3) vehicles before finally running into the sleeper cab side of a tractor trailer. (1) victim pinned in a minivan.

Frequency: Units Operating: YPD 1st PCT, 2nd PCT, ESU Truck 2, Truck 4, Truck 3, Car 851 (ESU LT); NYSP, YFD E-314, 311, L-70, R-1, B-2, Car 5; Stat Flight Air 1; Empress, Eastchester VAC 57M1, 57B4, 57B2 Description Of Incident: Vehicle off road into woods with pin and ejection. Writer: Truck4 (O/S), ems-buff 2200hrs- Units dispatched to rollover MVA on Sprain southbound under Plamer Rd. overpass. All occupants out of car on arrival. 2215hrs- Detective Division unit flagged down on Tuckahoe Rd near southbound exit ramp from Sprain by a female injured in an MVA. Dispatch checking with units at original MVA. 2216hrs- Female relating she was thrown from a white sedan. Truck 2 advising an SUV involved in first MVA. 2224hrs- R/C 101 req. to search Sprain southbound from Jackson Ave to Tuckahoe Rd. 2234hrs- R/C 101 has (2) pinned in the vehicle "way in the woods", just north of Tuckahoe Rd. 2237hrs- ESU Truck 3 on scene advising car a distance into woods with a possible water job. (1) pinned in car, (1) ejected into woods. 2238hrs- M/A Eastchester Vac 57B2 and 57M1 toned out for M/A to Yonkers to the Sprain for MVA. 2240hrs- ESU has (1) ejected victim approx. 50 feet south of vehicle in the stream. Truck 3 advising victim in car is partially ejected out sun roof. Vehicle will have to be stabilized. Units will have to bring equipment from roadway down embankment, through water and up into woods. 2245hrs- Yonkers FD B2 request to have Stat-flight to the scene. 2247hrs- Car 5 will be the ground contact for the Bird. ESU and FD are stabilizing car and setting up to lift vehicle. 2249hrs- Stat-flight air 1 in in the air 6 min ETA to the scene. L/Z with be ahead of accident on Parkway just north of Tuckahoe RD. 2257hrs- Stat-flight on the ground. 2300hrs- Stat-flight in the air Txp to WCMC 2305hrs- Tac-3 Enroute with (1) BLS to WCMC 2315hrs- 57B2 and 57M1 will be on scene for a while if more Patients are found. 2320hrs- 57B2 and 57M1 are 10-8 as per B2

Reported to have serious head trauma and transported to Cornell Trauma under NYPD escort. NYPD and FDNY conducting A.I. as of 2130hrs. M/A 3rd ambulance from Mahopac Falls. 2110hrs- Mahopac and Mahopac Falls EMS toned out for ambulances to scene. Somers CMD req. they expedite. 2113hrs- 46.14 for Stat Flight to ground contact. 2116hrs- Stat Flight Air 1 on ground. Air 2 with 12 minute ETA. CMD req. Mahopac Falls and Mahopac Ambulances to enter from Route 118 due to traffic. 2117hrs- 19-7-2 with 3 minute ETA. CMD req. 19-7-2 to back into scene and load PT for transport to Air 1, already on ground. 2119hrs- Somers CMD reports traffic problem at Miller X 202. Req. State Police or Fire Police shut road. 2126hrs- 19-7-2 enroute to LZ with PT. 2129hrs- Air 2 on location. 2nd PT is loaded and ready at LZ for Air 2. Air 2 will land after Air 1 takes off.
