L-27 FAST. 1338hrs- B-17 transmitting 2nd Alarm for fire in a 1-story taxpayer. 1244hrs- B-17 rpts (2) L/S/O fire is D/W/H. 1300hrs- Div. 6 rpts a partial collapse of the building with members trapped, numerous Maydays. (1) firefighter down being removed from the front. Transmitting 3rd Alarm. Staging Grand Concourse X 172 St. 1310hrs- Maydays still being transmitted. (2) members have been located in the basement, roll calls being conducted. Multiple NYPD ESU enroute to asisst, including Adam-3, Boy-3 and Truck 3. 1315hrs- S/C additonal TAC unit and Photo Unit. 1318hrs- S/C Bronx Collapse POD from Randall's Island. 1319hrs- Req. additional Squad, Rescue 1, L-25/Collapse Rescue 1 to scene. 1321hrs- S/C (4) additional Trucks to scene (55, 17, 48, 58) 1325hrs- Reported to be (5) missing Firefighters ATT. 1329hrs- B-33 is Air Recon enroute with (5) minute ETA. MSU #1 to front of building with as many 1-hour bottles as he has. 1346hrs- S/C Mobile Command Center and Rescue 5 to scene. 1347hrs- New staging area for incoming units is 173 X Walton. 1348hrs- EMS transporting (1) member to Jacobi, with PD escort, with serious but non-life threatening injuries. 1351hrs- Sq-252 relocating back to Brooklyn. 1353hrs- Car 1D responding to Jacobi. Sq-270 responding to Randall's Island with keys to access the POD. 1401hrs- Car 4 rpts (2) members being removed from the building ATT. Still have (2) members trapped with co's with them and giving them air. D/W/H. 1404hrs- E-91 responding from Randall's Island with POD. 1409hrs- NYPD ESU U-5 cancelling all additional ESU Units not already at scene. 1412hrs- E-22/High Rise Unit responding to scene. req'd to Walton X Mt. Eden. 1420hrs- (1) Firefighter still trapped. (1) member being transported to Bronx-Lebanon in cardiac arrest. S/C additional Rescue (R-2) and Additional Sqaud to scene. 1422hrs- Rescue 6 being activated through SOC. 1426hrs- EMS making notification to Bronx-Lebanon for a 2nd Firefighter to be transported in cardiac arrest. 1440hrs- S/C additional B/C to scene and (1) B/C to Jacobi (15). 1458hrs- EMS rpts (1) firefighter removed in cardiac arrest has reportedly been revived at Bronx-Lebanon and is being prepped for transfer to Jacobi. Out of the (5) trapped firefighters, (1) remains in cardiac arrest, (1) critical and (3) being carried as serious but not-life threatening. 1500hrs- S/C (1) additional Truck for FAST (34), (1) additional SOC Support Truck and an additional Engine and Truck (45/46). 1530hrs- Rescue 5 released and relocating to Rescue 1. 1600hrs- (1) Member has been pronounced dead at Bronx-Lebanon Hospital. Initial reports, and subject to change: Engine co's were pushing (2) lines into the taxpayer when the floor collapsed. Most of the trapped members were from the first arriving engine co's. 1610hrs- Rescue 6 in service at Roosevelt Island (SOC Command). 1621hrs- Car 4 rpts all Primary Searches are now (-), Secondary Searches delayed due to structural instability. Date: 8-23-06 Time: 0430hrs Location: Orig. 22 North 3rd Ave Units Operating: Mount Vernon, Yonkers Description Of Incident: Armed carjacking of a bread truck, with driver robbed and assaulted. Suspect chased onto Moutn Vernon Ave where he rammed a MVPD Patrol Car, injuring an officer. Suspect continued into Yonkers and bailed out on Bronx River Road, where he was caught near 451 BRRD.
