BUT, there are cases where it is accepted such as: A stable patient who speaks a foreign language that you don't understand to help interpret. A stable child who is frightened to be away from their parent. ( I had a case the other day, a 6 y.o. child had a obvious fracture of the ulna/radial bones. The situation was very stable. The child never even cried!) Possibly one other reason that I can't think of but that's it. otherwise upfront or not at all (especially if critical and they have no alternative way to get to the hosp.) That's about it, but at all times it is the medic / crew chief's final word.

That is not always the case not every psych call is due to a crisis team. Some call on thier own. When the pd is on the scene and wants the patient transported, unless they were directed and or going with you to said facility you should be going to A.) the patients choice (if the patient is capable of makeing such a decsion, most that I delt with are.) B.) If the patient is not in sound mind to make a descion you go where the pd or crisis team sends you. or It is generaly the EMS departments protocols that if a patient is not capable of making a descion, and the PD doesn't care, they go to their counties mental health hospital providing distance is not a factor.

The hosting department should have a shuttle service available for all parade participants. They should also note in the parade rules that they will not tolerate such an act. Why? Because if such an incident (I hope it never does) were to occur, I'm sure there would be some type of legal action against the host department for not providing such transportation. If such a service and advisement is provided the legal liability of the host department would be a little less (lawyers, they know how to get you no matter what.) I'm sure deals can be worked out with your local school or bus system, and this may save a life or injury.

If a Pt. is handcuffed yes a police officer should be in the back with you. If the Pt. is secured with flexcuffs, and you still don't feel safe then demand an officer ride. If you are comfortable, your trauma shears work just fine for removing flex cuffs. As far as cops willing to go to the hospital with you, we have a department who 95% of the time will not go with you because it takes 2 officers (1 in the back and one to return the officer in a timely manor) out of the area for about an hour. (They also complain about being understaffed.) So who gets stuck with all the unnessacery BS? The cheaply paid (or volunteer) EMS worker.

Times Herald Record July 11, 2006 Injured Monroe firefighter taken by medivac A Lakeside Fire Company firefighter was taken by medivac helicopter about 30 minutes ago to a Westchester hospital after he was injured at the Trout Brook Engine and Hose Company in the Chester Fire District. Details as to what happened were unclear, but one assistant fire chief reported that a vehicle backed into the unidentified firefighter, who has been with the department for more than 25 years. The firefighter, who was said to have suffered a leg injury, was taken to Westchester Medical Center in Valhalla. He was treated at the scene by Mobile Life and Chester volunteer ambulances. Monroe firefighters were on stand-by at the Chester firehouse as Chester firefighters were responding to a fire call on Route 17M near a sports complex that is under construction.

First off, if you are tightening the straps that tight then you are definitely physically abusing the patient. Second, patients can be calm and show no signs of struggle and then suddenly go nuts. We have all witnessed this or just the opposite they can be crazy and then once you get away from everyone calm right down. In any case if you feel the patient is a risk, have the cops zip tie them to the stretcher. This way if you needed to you can cut them off. just as good as handcuffs! Using pepper spray in the back of the bus is just going to piss them off and irritate you and your partner that's driveling. And I agree 100% keep your eyes on your patients! They can go nuts on you or they just may crash on you suddenly. The crews safety is first priority over patient care!

A medical helicopter can be used for certain medical conditions that have to be treated at certain hospitals. This is usually with the approval of local medical control who will suggest the method of transport if the facility is not within a quick transport to a the facility. Yes it is true that for the lower Hudson Valley we generally have 3 helo's available, but we do have the ability to call (depending where you are) for a chopper who is out of state (if none are avail. and LifeNet determines it's available. This of course would need the careful consideration on if you can make it to a level 2 center before one of those choppers to arrive at your scene.) Stat Flight Air-1, Air-2, and the NYSP choppers cover Westchester, Rockland, Putnam, Orange, Dutchess, Ulster, Sullivan and sometimes Green Counties. But have been called to other areas when nessacery. There is also the fact that if the State Police Helicopter is used for a technical rescue a 2nd helo. may be needed for transport to the trauma center. (obviously depending where you live.)

That being said, Has anyone noticed a dramatic increase in medevac missions in the Hudson Valley? And are they being utilized to their full potential?

My department's (fire / ems) allows volunteer's to go to the scene. On the fire side one large reason is our commissioners don't feel we don't need anything more than a 2 man cab. We have instructed our firefighters to use thier heads when parking and to park as far off the road and away from the scene as possible. On the EMS side, an EMT is allowed to meet the bus on the scene if they are not already in the building.

That's why it is also very important for your training officer to document all training. If it's not written, it wasn't done.

I think the only time you might get away with it is loading hose at a very slow speed.

I recently had by BC245XLT repaired. I sent it directly to Uniden, about $65. I had it back in about 2 weeks. They sent me a new battery too! I was very happy with the service. (make sure when you send it to get a return reciept and insurance is a good idea too.)

This was a very touching gift that ABC and the community gave to this family. The people of Westchester County and all others that helped make this come true for this family should be proud of your selves. GREAT JOB FOLKS! Was it me or did Ty look a little exausted at the end of the show?

Use some simple math, just because they don't have a 6 or 7 story building doesn't mean that every 3 or 4 story structure sits next to the road. They may be 30 feet away from the building, now add the elevated hight of the structure, now move from one end to the other. Do you have that much extra? I don't live in an area with too many tall buildings but that doesn't mean that every ground ladder will reach the roof. 100' ladders can be very useful like when you have a very long building fully involved and the only way you can get water on it is that nice tall ladder! Do they need to cost a million plus, no they don't but that's a different topic.
