Expounding upon SOAP notes- which breaks down to Subjective, Objective, Assessment, and Procedures- which is a staple in the hospital environment is the old prehospital elaboration of "CHART"- C- Chief Complaint H- Hx or History (present situational and pertainent past medical or traumatic) A- Assessment (pertainent findings) R- (RX) Current Medications (if pertainent) T- Treatment Has never failed me- and I loathe talking on the radio to the hospital- not to sound all glorious and covered in all things tasting of salt, but I have better $h.. to be doing in the back of an ambulance than telling a hospital a succinct report of what I'm eventually going to tell them in another "5 to 10".

+1 on the bleche white, or spray 9 to pre clean- but don't do it all of the time (bad for tires). Do each tire individually- spray on, let it soak a second, and scrub like hell with a soapy scrub brush until you get a brown foam coming off the tires, then spray off and dry with a towel. Don't forget to get the wheels and wheel wells with a non-harsh soap and a wet towel Once dry if your the type to paint the raised letters on the tires ( on fire apparatus ) white with a grease pencil do it before application of any shiny garbage and let sit for 30 minutes. THE ONLY "tire wet" product I use is Stoner More Shine- it produces more of a classier even matte shine and will not gunk up your wheel wells. Plus if you need to change your tire it's not really greasy as opposed to the alternatives.

EMS BACKGROUND; Paramedic Mark Bedell has been involved in Emergency Services since birth, as his Mother and Father were both Emergency Services Coordinators in Rennselear County, NY and held connections to the Big Indian-Oliverea Valley in the Town of Shandaken. Mark has been a NYS Certified Paramedic for 20+ years, and has been involved in EMS for 30+ years. In addition to his military service, Mark has served for numerous EMS and Fire organizations in countless leadership roles in his travels while holding technician level or instructor level certifications in most technical rescue, firefighting, and EMS certifications available within New York State. In addition to currently working a whopping 72 hours a week for Shandaken Ambulance, Mark currently serves as a Captain and Commissioner of the Hoags Corners Fire Co. No. 1 in Rennselear County and is also a member of Hoags Corners Volunteer Ambulance Squad. REASON FOR AWARD NOMINATION; On top of working an incredible 3,700 hours as a Paramedic for Shandaken Ambulance last year, accounting for slightly less than a third of Shandaken Ambulance’s total Paramedic coverage, Paramedic Bedell had a particularly positive impact on patient care September 6, 2009. In the wake of a fatal recreational tubing accident one week earlier, At 1229 hours Town of Shandaken Ambulance along with the Phoenicia Fire Department, Ulster County Sheriffs' Dive Team, and Shandaken Police Department responded to a report of a tuber in distress on the Esopus Creek. Upon the quick arrival of Fire Department first responders, Town Police, and Paramedic Bedell it was determined that a female patient had her leg wedged between large rocks in heavy rapids approximately 60 feet from shore, was visibly exhausted, and yelled to shore that she “could not hold on any longer” . Without any other effective means of reaching the victim and utilizing his technician level training, Paramedic Bedell selflessly affected a go- rescue with the assistance from the Phoenicia Fire Department. Paramedic Bedell successfully disentangled the individual’s leg from the rocks, provided aide to her until she could be extricated from the stream by the Ulster County Sheriffs' Dive Team and Phoenicia Fire Department, and transported the patient to the hospital. Just an hour and a half later ( and before Paramedic Bedell could dry off ) Shandaken Ambulance as well as Phoenicia Fire Department and Shandaken Police Department were dispatched for an allergic reaction on a hiking trail off of Old Plank Road. Upon arrival of the Ambulance (which was returning from the hospital) Paramedic Bedell along with his crew hiked approximately ½ mile to arrive at a patient in acute anaphylaxis secondary to multiple bee stings sustained on the trail while hiking. Following an administration of High dose Epinephrine, Benadryl, and a fluid bolus the patient was successfully extricated off the trail, and transported to the hospital with positive prognosis following. CONTRIBUTION / IMPACT ON EMS; Paramedic Bedell’s actions on September 6, 2009, as well as his dedication to the Shandaken Ambulance Service exemplify the qualities of an individual truly dedicated to his craft. His longevity in the EMS industry defy all statistical odds, and his dedication far supersedes even the most “gung- ho” of EMS professionals. It is without doubt that Paramedic Bedell will continue to serve the citizens of the Town of Shandaken, and beyond with the outstanding care, compassion, and enthusiasm that he has for the past 30 years.

Pretty cool, like a gigantic Ipod touch. The screen resolution is insane and I could see myself breaking it soon (although I'm sure some aftermarket cases are out already, one of which I should probably buy). The keyboard feature is horrible however beats a ipod touch or some of the smartphones out there. It would be a lot nicer if it had a USB for a printer/ other USB stuff. (Here comes the buff and capitalist) I wish I knew somebody that knew the programming well enough to produce an EPCR program for this- it would be a goldmine.

A. Did they get there? B. Did they serve the purpose that they were called for? C. Did they have adequate manpower/ equipment? D. Was there untoward effect (loss of life/ increased risk) secondary to their delayed response under extreme circumstances (ie. 3 extra alarm fires in the same Battalion)? E. Did that response/ utilization of resources/ specific department preplan save or spend manpower from a department that otherwise dedicates limited manpower to high volume areas? F. Would it be different if it was 1100 hrs on a weekday? These are questions that when answered will lead to a productive conversation, otherwise there is a lot of scanner listening - armchair quarterbacking in this discussion which seems to be an increasing trend in this forum. This is extremely counterproductive,leads to overall misinformation, and threads being locked. Albeit there are a few with tride and true fireground experience speaking of this call, most partaking in this thread (including myself and 90% of the membership here on EMTBravo) have somewhat limited fireground experience (just from age and profile information) in comparison to the scopes of decision making that is required in such unique situations (ie. practical preplans for multiple extra alarm fires in the same area with limited manpower/ resources and water supply coordination for a hydranted city during a state of emergency that essentially puts all of your hydrants out of service). Perhaps before we break out the "jump to conclusion mats" we could base our $h!t talking on some valid information. Perhaps "senior members", UC 911/ DC 911 Dispatchers, Emergency Management professionals, or a Chief of an involved Department would care to elaborate prior to this becoming a "blue light argument".

PCR update 12/30/09 from Lee Burns, Acting Director, NYS DOH Bureau of EMS From Lee Burns, Acting Director, Bureau of EMS on 12/30/09. I just wanted to let you know that the Department of Health is in the process of printing 1,000,000 Patient Care Reports forms. We expect delivery in the not too distant future. Once we get the forms, they will be shipped to our EMS program agencies for immediate distribution. Since the arrival of the PCR shipment has not yet been provided to the DOH, we have advised EMS transporting agencies that if they are interested in printing PCRs locally, they must print the approved DOH form and that they should only obtain a supply that will hold them for 60 days or so.
