Date: 7/4/12 Time: ~2145hrs Location: Long Island Sound off Larchmont Yacht Club (1 Woodbine Ave) Frequency: 46.26 (Dispatch), Fire 15, 155.775 (LPD), Marine 16 & 72 Units Operating: [Larchmont FD: 2222, E35, E34, TL7], [Mamaroneck Village FD: 2242, E42, M2], Larchmont PD, Mamaroneck Village PD, Sea Tow Weather Conditions: Hot, Hummid, 80 Degrees Description Of Incident: A malfunction of the fireworks caused a subsequent fire at The Larchmont Yacht Club. There were 2 rounds that misfired and exploded on the deck of the barge. Following the initial malfunction, there were 4-5 more misfires that exploded at or just above the deck of the barge. During this round of misfired shells, there was a bright red glow on the deck of the barge. The situation was mostly stabilized prior to the arrival of PD and FD boats. Date: 7/4/2012 Time: 0619hrs Location: I-95 SB (between exits 15 & 16 @ mm 4.8) Frequency: 33.96 (Dispatch), EMS 18, NRFD, NYSP, FDNY, 155.130 (NRPD) Units Operating: [New Rochelle FD/EMS: 2302 (IC), E21, E24, L12, R4, 30A1 (EMS Command), 30A2, 30U1 (MCI Unit)], [Transcare EMS: 921 (Transport Unit), 807 (Supervisor)], [Eastchester VAC: 57B3, 57M1], [Larchmont VAC: 66A1], [Mamaroneck EMS: 68A1], [FDNY*EMS: 1 ALS Unit, 1 BLS Unit, MERV 2], [WCDES: EMS 10], NYSP (Thruway Units), NYSTA, Vincent's Towing Weather Conditions: Light Rain, Some Fog, 70 Degrees Description Of Incident: Tour Bus into the guardrail, extrication required. Driver possibly ejected from vehicle. 24 total patients, all were transported. Bus was coming from Foxwoods Casino in Connecticut. NRFD units handled other calls in New Rochelle. Date: 6/30/2012 Time: 1628hrs Location: Taconic State Parkway SB (@ Underhill Ave Overpass - mm 13.8) Frequency: 46.26 (Dispatch), Fire 17, EMS 17, YPD Units Operating: [Yorktown FD: 2532, 2539, E273, R16, MA8], [Milwood FD: 2252, E247, U44], [Yorktown VAC: 86B3], [Ossining VAC: 74A1], [34 Medics: 34M1], Yorktown PD, NYSP, NYSDOT Weather Conditions: Hot and Hummid, 90 Degrees Description Of Incident: 1 Car MVA rollover with 1 patient ejected approx 15 feet, all SB lanes closed. 2 patients transported ALS to WMC by YVAC and OVAC. Millwood FD units were released after patients were in care of EMS. Yorktown units remained on scene to assist with towing and fluid mitigation.
