  1. I can't think of any engines that have sacrificed hose bed height for extrication equipment or other toys. It has always been to accommodate more water. I think thats a more appropriate question. Why do we suddenly need every engine to also be a tanker? I know Pleasantville went this route because our hose lays to our low water areas are difficult at best to accomplish, and during the day almost impossible.
  2. Its not going to do anything to your score slick, but if as GFD says and 30% fail then a placement of 9,000 is as good as 6,000 or 3,000 equals 2,000.
  3. A few have mentioned a smoother ride in a type 3. Could that be a difference in manufacturer. I've noticed that the Chevys all ride smoother than a Ford but I didn't notice a real difference between the two Chevys.
  4. Drive one everyday down in the Bronx. I wouldn't trade it in for a type lll any day. I've got alot more cab room especially with the MDT and radios. You lose a little in the turning radius and have to creep out a bit more carefully into intersections, but the extra safety afforded by having all that mass out in front of you is a great bonus.
  5. Don't let these companies fool you guys. There are a few that are barely keeping their heads above water, others that are doing quite well, and others that are rapidly growing. 911 is a money loser, no doubt about it. Thats why you don't see commercial agencies that just do 911. Transports make money. If they didn't then how do you explain, how Transcare, Empress/Emergacare, WEMS (I know WEMS is a not for profit subsidized by Stellaris but even they are not free to just lose money) survive in Westchester. They don't even have all the transports in the county as agencies like Empire, Lifeline, and Senior Care handle some.

Other communities in Ohio are treating and releasing. Maybe some day before I retire it will come to NY.
  7. For a couple of reasons, MRSA has been an issue in nursing homes for 10 years now. Its gotten more common in that time and there is some worrying news about how the bacteria has changed, but there is nothing to justify the recent panic being fed by the media. We should be treateing every patient contact like they have MRSA because we really don't know what anyone has. That means gloves on every job and handwashing after every job. If there isn't a sink available then get out the skell gel (antibacterial gel). Wipe down equipment that has touched the patient (scope, BP cuff, stretcher, etc) with alcohol or any other anitseptic wipe. This is all about the safety of redundancy. Truth is MRSA is a relatively low risk exposure without coming into contact with fecal matter, puss, or saliva. A little bonus that I picked up on the other day...even fido can give you MRSA sp just be careful and wash-up.
  8. The current vest available to EMS is stab and bullet resistant. Oddly enough they were doing a fitting today and someone else brought up why the inside of their vest says "not for ballistic use". Our vests are two layers, the ballistic and stab. I don't have the info in front of me, but from what I remember the vests Balistic level ll and level 3 stab resistant. In their testing the addition of the stab vest brings the level of protection up to the equivalent of lllA but its not certifiable since they are two separate layers. There is a higher level of stab resistance, puncture resistant that this vest is not.
  9. What an arse. There's never a reason to put hands on someone unless they are preventing you from doing your job. Even then, you'd better be pretty damn sure of yourself before you do. Maybe I'm just lucky, but I've never had these problems. Roll up on scene of an MVA and I go to who ever is operating and ask whats going on? There's always plenty of work to go around so there is no reason to not play nice in the sandbox.
  10. Its not about how much A-Rod wants to be a Yankee, its more about how few teams are willing to pay for him. During the GM's meeting the only teams that even appeared interested were the Dodgers, Angels, Giants, and one other that I can't remember. Of them only the Giants could afford him and had a need for him, but they aren't anywhere near contending next year. I say let him come back at no more than the average for the top few at his position. There are other players capable of replacing him.
  11. NYPD ran into trouble with the Segways. The broken and uneven surfaces of city streets were causing guys to fall, so now they've switched to a three wheeled version.
  12. Here's something radical. How about no benefits if you are not wearing your seatbelt during an accident. We all know better, and many of us will go out our way to turn off that annoying seatbelt warning every time we get into our vehicle.
  13. When I went to school there it was common for crews to cross the border in both directions. From the NY side, the county would call ahead and announce which vehicles would be going and a lane would be prepared for them. They would breeze right through and then have to go through a normal crossing interview on the way back. There is absolutely no reason for this delay. The Canadian Border is such a joke. There are established fire roads, snowmobile trails, and trails that cross all over the damn border.

  15. Everything I know about Hemostatic Agents is complete hearsay. Its not discussed much and I had little motivation to pursue much info as it didn't look like they were coming anytime soon. The heat was an issue brought up, but the numbers were in excess of 120. Enough to potentially cause thermal injury. They made debridement difficult and were contributive in many post-op complications. Overwhelmingly negative may have been a bit of an overstatement, but in this area with transport times being less than 20 minutes to a Trauma Center in most cases and often only a few minutes to the nearest hospital it seemed unnecessary. So without ruining this portion (I'm waiting for the DVD) of the "ALS Presents CME Series" what am I missing? Or just give the whole thing away and bill Seth.
  16. A while ago a NYC*EMS EMT was stabbed by a pt and his personally purchased vest was credited with saving him. Since then body armor has been available to all members. Catch is you have to go out to Queens when the vender is there for fitting. Only a few wear them every day. Events like the West Indian Parade and certain neighborhoods get more guys wearing their vests.
  17. The Cushmans are out. There are still a few kicking around but I haven't seen them used. The Gators can go more places and transport an immobilized pt. The biggest hurdle for the Harley program is the lack of available paramedics. There are about 80 medic students getting ready to graduate in November and they are expected to fill all of the vacancies in the system right now. The current schedule of medic programs is suppose to cover attrition and expected expansion. So for the time being there just isn't enough manpower to add something like the bike medics. At this point the city is better served adding more transporting units.
  18. The money being paid out per trip is significantly less than it was just a few years ago. The profit comes from the increase in the number of transports. Sub-acute care is increasingly handled outside the hospital. After joint replacement people use to spend days or even weeks in the hospital until they were well enough to go home. Now as soon as they can be moved people are transfered to a rehab facility. There is also an increase in the number of people who will be using the services between the growing number of elderly and advancements in medicine.
  19. crcocr1, re-read what I wrote. Transport agencies are making more money than they have in the past. The money is so good that there are a even a few municipal agencies who have started in on this territory. My point is that while the agencies are pulling in more money it is not trickling down to the employee. You highlighted my comments about how the EMT is not making any more and would be better served working somewhere else.

  22. Every EMT has heard about the risk of suppressing hypoxic drive with O2, but recently I heard there was research suggesting supplemental O2 being withheld during cardiac arrest and CVA. Apparently the extra O2 suppresses the body's efforts at preserving undamaged tissue. This is the first I've heard of this and don't have access to any of the journals from my lowly comp. So until I can get Manhattan to go journal diving is there anyone here that can help me out?
  23. With so many lights and reflective striping I find it hard to believe that vehicle color has anything to do with visibility. If it is such a concern why have I seen so many PD command or Bomb Squad monsters black? Pick a color that works for you.
