  1. I'd heard 343 was the first boat and headed to Marine 1. Build completed by early 2010 sea trials and and delivery by the end of the year. What changed?
  2. I've seen too many living people receive CPR and anxiety attacks or URIs receive ibuprofen, naproxen, and acetaminophen. These instruction are only as good as the information the CRO receives. GIGO, garbage in garbage out. At least in these cases there has been no real harm caused. You give epi to an 80 year old in ape you really could kill him. You give it in the wrong location and catch a vein (I know, its an outside shot) but again you've potentially done harm. How many Anaphylaxis patients are found in respiratory arrest by arriving EMS units? That would be easy enough to find out. Its time for fact based changes in EMS. Not just "ehh seems like a good idea."
  3. Infrared behaves much like visible light in that its absorbed, reflected, and deflected by glass. While it could work from inside a vehicle it would diminish the quality of the image it reads especially on plates covered by glass or plexiglass.
  4. Am I correct that is just a rewrite to include all the changes of the last 6 years? Nothing new since c-spine clearance, correct?
  5. Date: 4/12/09 Location: Houston, Tx Departments: Houston FD Description: Two Houston firefighters died battling a house fire in southeast Houston early Sunday. The firefighters were called to a fire at a home on Oak Vista and De Leon streets a little after midnight. Firefighters were inside the home searching for victims when they were called out because the fire became too intense. But the two did not make it out. http://cms.firehouse.com/content/article/a...mp;sectionId=39
  6. Don't you read the Post?? You're a retired cop, you're loaded living high on the hog. We all know you have your pension funded luxury box.
  7. unions would absolutely go a long way towards fixing the money problems in EMS. Without it, why would employers pay more for someone with more experience when they can just keep on hiring the latest tech out of school for many dollars an hour less.
  8. citi asked for their money back, but the mets said no deal. As long as citibank is solvent they are stuck in this deal for the next 20 years. The Mets even spun it as its a good deal for citibank and the taxpayers. O drive between new and old Yankee stadium every day to and from work and its reall sad watching the old stadium slowly come down. The new stadium is gorgeous and the playing field is almost identical to the old stadium. They did a phenomenal job of combining old and new. Its just brutal what they're charging for tickets.
  9. A few things, gun control does not increase or decrease crime. Look at Baltimore and DC. Statistically 2 very similar cities until DC instituted their weapons ban. Shortly after there was big spike in a variety of violent crimes. At the same time Baltimore went through nearly identical increases. So was it the weapons ban or was it something else? The assault weapons ban was about accessories to guns. High capacity magazines, pistol grips, etc. Fully automatic weapons are still illegal. The AR-15 was still legal to own under the ban and most semi automatic riffles can fairly easily be converted to full auto. In those two incidents NYPD ESU even if they weren't distracted by MVA's and crane collapses still wouldn't have made a difference. The damage was done before the officers on scene knew what they were into.
  10. This isn't about the pump or supply hose. In areas where hydrants are plentiful, who carries a few lengths of attack line and a knob for those few occasions where you might be stuck twiddling your thumbs at a car fire or rubbish fire waiting for the engine? This wouldn't be for initializing an attack on a structure fire, merely attempting to extinguish a fire before it could grow or spread to a structure. Do a little more than dump 10 gallons of water from the can in futility.
  11. I'm confused. We're talking metal studs on the first floor and wood studs on the second right? Not floor joists. Are these just interior walls or load bearing walls?
  12. Its a valid complaint. Every restaurant I've worked at knew it was going to be dead during certain events. The smart ones found a way to capitalize on it. Besides, if those two days are the difference between success and failure you're issues are much greater.
  13. When you talk about making a truck sing, someone who knows their truck working the gears with just the throttle can really move and it is 100% by sound and feel when done right.
  14. I never understood the mirror lettering. Are people going to pull over because its an ambulance, but they would have told a police car of fire truck to go screw?
  15. It absolutely costs. At the very minimum its to clean up after the come rolling through. There's also the issue that these people do often have legit medical conditions for which they receive no primary care. In that respect they're just like most of everyone else crowding the ER. Stories about 9 people going to the ER thousands of times are year are great and all, but it still doesn't address the problem. An ounce of prevention is worth a pound of cure.
  16. Recently the city hospitals have been consolidating and eliminating duplicate medical record numbers and have discovered some totals that would bury those 9 in Houston. Some of these people had thousands of visits under misspelled names and many time more under their actual name. However I'm not so sure the cost to citizens is accurate. The majority of the time these frequent fliers enter the system they just sober up and leave often from the waiting room.
  17. nice photos. Anyone have a play by play?
  18. http://jacobitrauma.com/ The Annual Warren Wetzel, MD Trauma / Emergency Medicine Symposium represents the only regional conference dedicated solely to the management of trauma patients. As such, it has been able to attract a nationally recognized faculty. New York City has 20 trauma centers, and the Symposium provides an important forum for the sharing of science and ideas related to trauma. Program Goals Physicians and nurses should be aware of the many complex management strategies available for the treatment of trauma victims in order to minimize complications and mortality. The educational goals of the Symposium are for the attendees to: Gain awareness concerning traumatic brain injury in the professional athlete Review current diagnostic and management practices in the penetrating neck trauma Recognize and understand the key issues in wilderness and environmental medicine Learn about and understand current controversies regarding the management of abdominal trauma Learn and become aware of new algorithms in resuscitating the blunt trauma patient Gain awareness of key issues central to managing the obese trauma patient Explore how trauma systems affect trauma triage protocols Introduce the healthcare professonal to related issues in gang violence While it is largely geared towards in-hospital issues there is always plenty of good info for use in the field and it looks like there will be even more this year as more and more EMS personnel attend. It is approved for NYC REMAC CME credits, not sure about any other regions.
  19. Yeah, too bad it only applies to commercial and industrial structures. In residential, where the fire protection standards are less rigid and truss use not as obvious we're still SOL.
  20. If these 2 boats can be used to establish a year round presence on the Hudson, then it looks like a pretty reasonable expense considering the money is a homeland security grant. To the south, you have FDNY and NYPD who are most likely coming from lower manhattan To the North, what FD or Police units are in the water and staffed year round?
  21. I'm confused here, why delay drugs for a tube? So what if the belly is distended due to some over zealous ventilation. Get them doing ventilations the right way; one on the seal and one on the bag, squeeze until chest rise, once every 5, etc. If you're not shocking, administer first line drugs and then while you're waiting three minutes set up your ETT and during the switch/rythm check drop your tube. Yes, it will probably delay the second round of drugs for a minute or two but it gets everything moving a little bit faster over all. Especially if its a tough stick, or a tough tube. Once the tube is secured you can burp that over inflated stomach and take some of the pressure of the vena cava. If BVM is ineffective then absolutely, then airway is a priority and you grab the tube kit. All this being said, there's no reason to transport without an advanced airway in place (excluding extremely unusual circumstances). A while back I ran through just about the entire drug bag on an arrest, and finally got to intubating about 15 minutes in. Ventilations were great and I had the capnography strip to prove it. I'm either going to need a new partner or figure something else out once we get our Res Q Pods. Need a tube for those to work.
  22. cooling therapy is great, but useless without the police officers who got on scene first and the EMT's and paramedic who did the hard work to get her hart pumping again. Good job guys. Enjoy the spotlight.
  23. leds, wig-wags, strobes, etc don't move traffic. You could mount enough lights to give pedestrians a tan as you pass and they're not going to move traffic. Driving technique and siren use will move what little traffic that is willing to move.
  24. rescue40, now that is an incident worth posting. Sounds like you guys did a good job.
