Modern day healthcare.....especially the motto of Westchester County area EMS........."Profit Over Patient" Whatever happened to "To delivery quality healthcare regardless of economic circumstance"????

Just curious as to if any agencies in Westchester are using/considering using C-Pap??? I feel it's an extremly useful tool, coupled with Nitrates, in the treatment of CHF. Most often, it resolves the CHF episode and the patient is sent home 1-2 days afterwards. Many progressive agencies nationwide are begining to use this device, which is not a hard one to learn. With CPAP, many agencies also take Morphine out of the protocol. It decreases preload, but it also can lead to repiratory depression which is leading those agenicies to pull it. The only downside is it consumes a large amount of Oxygen. Your ambulance has to have dual mains to be the most prepared for it. Why can't patients in the WC have the latest and best? Why must they settle for the current standard? Why can they travel 10 minutes from the WC and get more advanced ALS care?

Take nasal narcan for example. An easy addition to our toolbox. Just add an MAD. What is the issue with that? Easy to administer, just like a nasal spray, and reduces risk for needlestick or bloodborne pathogen transmission.

Code 3 did make the Mack CF Engine 306. I'd like to see the current ALF Engine 306 (They already have the mold, and Past Time Hobbies released a simlar exclusive model) But before that, where my Yonkers REP unit at???

I didn't really like the first episode,and was very disappointed with it, but I'm willing to give the second episode a shot tonight. Sometimes shows get better after the first episode.

Several studies have been done proving the effective use of CPAP both in the pre-hospital and ER arenas. I have also witnessed its effectivness personally. As Skooter said, we need to prove that we are "worthy". Unless systems get behind their medics, do QA/QI and educate (Mandatory CMES's/educational reimbursment) inservices, etc), then we're going nowhere. Let's not forget......behind every strong medic is a strong and progressive agency, strong educatiors, even stronger managers, and strong funding, and of course strong medical direction. Unfortunetly, in Westchester County, we don't have the support or funding we need to do studies or research,or provide more than basic education and equipment to EMS personel. We can't even communicate or have a working radio system to contact medical control when we need them. I agree strongly with the RSI comments. You can easily kill a person with RSI, IF DONE WRONG or you are overconfident. Westchester County does not have the resources to support such a protocol, IMO. That's a whole seperate discussion.

Westchester is just as "frugal" when it comes to ALS. At least they have two medics for the city, lol!

Does anybody have any further information regarding this program? Who is getting the trailers, what kind of equipment/setup will they have, who is responsible for the maitenence, what will tow the trailers?

In response to HFD211, some great points made. Still, you've got to consider the there are less fires nowadays and more of other types of emergencies, so FF's, in a sense, must be able to be a "multi-tool" of sorts. Of course we should focus and maintain our core skills, however we must look at our expanded scope as well. I think every department in this county should have a specialty, and learn to share and work those specialties into the mutual aid plan. As far as I have learned,these trailers are decontamination trailers, meant for the decontamination of victims. Basically, a mobile people wash. These are totally seperate from the roles of the Squads. Ardsley is also a recieiving center for Indian Point evacuation, so that's why I think they are getting one. Personally, IMO, I think Decon should be an EMS role. I'm still curious as to what will tow these trailers.

Oh, I've worked (ALS) in all of Somers EMS ambulances . My favorite is 80-B-2 because its the nicest looking,lol. The PL is nice, and I really don't mind the Hortons either. Sometimes I do feel badly for anyone on a backboard though, lol. Alot of the ride, IMO, depends on the driver, and my only issue with the Hortons B1/B3 is the lack of grab handles available. The ones that are up there are too short or non existent. When you're working byourself back there enroute, and trying to start an IV/do a twelve lead/intubate/assess/etc...there is hardly anything to grab onto or brace yourself against. In some cases, it's also nice to have all that room, but sometimes that room can work against you. Some agencies swear by their Freightliner/Internationals. I wonder if the suspension has anything to do with it? I've ridden in Boston EMS's GMC 4500/Brauns........the GMC 4500 is the equivelent to the Freightliner or International chasis, and it rides pretty nicely, well as nicely as it can along Boston streets,lol, with the suspension they have on it.

A good amount of progressive systems have it. Unfortunetly, with RSI, it's very dangerous when placed in a weak system or with an inexperienced,overconfident, or just plain weak medic.
